Reciprocal action

What do you do when thing doesn't go your way?
You react, you wait, because the world is round, what goes around comes around.

i was afraid of dentist, now people are afraid of me.

I was afraid of dentist that i didn't turn up at my appointment,
now my patient failed to attend to their appointment.

I just need to deal with the level of frustration........

Tomorrow is a better day.

Invisible aifen

There is a chinese verse saying "if the needle doesn't prick into your skin, you won't feel the pain". Yes, my patient had 10 carpules of local anaesthetic solution this morning for her full lower teeth clearance. This poor lady can't even swallow and spit water, not to mention about speaking. And, she can't write. This is the time when eye contact and non-verbal communication play their role, i can tell that she feel awful. Yet, i did my best to help her out of her condition, sincerely hope that she can cope with the rest of her life smoothly. There are always things that you can do nothing to help, she just has to live with that.

And then, we were in this super busy out patient clinic that two consultants failed to attend. The only oral surgeon had to see all the patients. Guess what, i just discover my new side kick power, i can make myself invisible. HAHA.. Well, he is too busy to even acknowledge my attendance, may be i should say existence. Oh dear....

There were time that i thought my intelligence level was quite good, but today i hang my washing out at night and let them get wetter with the heavy rain. Yes, i guess i am "smart".

This is the real inheritance, probably not so much genetic, more to life style and thinking. Do you believe that?

Hesitation

Hesitation, stubborn and Procrastination lead to a complicated problem with poor prognosis.

Eventhough professionalism is just about inequality in quantity of information, if you do not wish to spend the time that professional has to spend, deal with them, accept their suggestion.

FREE gift

Have you ever got free gift from any commercial dealer or lucky draw or even credit card?Is free gift a common marketing strategy now? Yes, it certainly is. And, you can easily see people go blindly for the promoted item, be it credit card or food, just for the free gift. But i guess no one actually assess the quality of the free gift, to whether they are worth the price and effort.

Personally, i have not believe in free gift anymore.
1) The free "i digital handy cam" that my dad got from one of the credit card company only work for one week before it has broken down. And, apparently none of the camera shop that we go can fix it. So, the 1st free gift is down and dead, lying in one of our cabinets.

2)Many so called "free gift" become garbage when it comes to house cleaning before Chinese New Year.

3)I bought a digital camera recently and the Miss Boss said she would give me this mini tripod as a gift. One of the leg of the tripod just broke when i first use it.

I cannot tell whether these are coincident cases that happened to me Only or there are many other people who had similar experience. Please, do not give me free gift anymore, be practical, give me discount or cash back. :/

post-op

Have you ever received any complaints from your granny or your loved one who wear denture, saying that the food you buy or cook is too hard for them to chew? Well, if you do and you think they are just trying to mess up with you and make your life difficult, you have to change your mind. I strongly suggest that you go for a complicated extracted that require suturing (eventhough you don't need one, LOL). It is only after this that you will realise how many foods that you think they are impossible to be considered "hard" are actually hard for the denture wearing people. Even grilled chicken can irritate the gum , not to mention about nuts and vegetables. At best, they will swallow what you give them in order not to dissapoint you and waste your effort. So, please be sensitive.

Prepare food that is chewable, make them into small chunk and ask them to chew on both sides. Also, ask them to take their denture out everytime when they do not need to use it. It helps in my granny's case. Presumably, it will work in most case.

Wisdom tooth

I haven't being injected with IAN Block after 2nd year in dental school when we learn how to jap each other. And, i have really forgotten about the feeling of being the one sitting down in the dental chair, waiting to be injected with the long needle.

Today, i got my first IAN block plus multiple infiltration all around my impacted third molar. To be honest, the injections weren't very pleasant. Hence, i would like to say thank you to all of the patients or colleagues that i have given injections in the past three years. You were really brave.

I am glad that i had the opportunity to experince what the patient felt during extration of impacted third molar (wisdom tooth). In the mean time, i feel really awful to have to gone through extraction eventhough i didn't eat tonnes of sweets and i was brushing and flossing my teeth. Well, this is life, life is unfair. Now that i understand so deeply that "agenesis of wisdom tooth can be a blessing", it really mean something to me now. And, i doubt the presence of an impacted wisdom tooth will make you any superior in term intelligence as compared to people without one.

Having my wisdom tooth extracted, part of my anterior border of my ramus drilled away with bone drill, my retromolar pad cut open and stitched back, I now have to swallow eat soft diet with small size spoon and not to mention about pain killerssss. It is hurting even when i swallow, I WANT THIS TO GO AWAY ASAP. :(

What do crabs eat?

Went to crabbing with some friends a few days ago, but unfortunately we didn't catch any crab, but a tiny weenee baby crab wondering in the net that we had to let go. So, there was nothing much to blog about that, except for beautiful scenary, strong wind, other fisher and jetty that was filled with dried fish bone and ralae. Kinda messy actually.

But the topic "what do crabs eat" came up on the dinner table. Between, we use kangaroo back bone as the bail for crabbing.

C:"There is no kangaroo in the sea, that is why crab don't eat them...., may be we should use dead rotten fish....."

W:"But some fishes may eat crab, so what kind of fish should we use?"

I: "May be we should ask the fishmonger, they must know? "

Hence, the conclusion is crab eat dead matter, i think any junk will attrack them if they are in the mood of eating and being catched. Aw, i am craving for crab after the dissapointing crabbing trip. Miss my mum's chilli crab so much now, , can't wait till 3rd of dec.

无知是幸福的

在书上读的疾病,癌症,死亡等等等等,
我真的以为这些都是书上的资料和"别人"的遭遇,
是我知道的比较多了, 还是"坏"的频率提高了?
好象我所读所闻都渐渐出现,老实说:我一点都不喜欢这一种感觉;

在desperate housewife里, Susan说了一句"everyone got cut up everyday, i think i can handle it." 这一句话被我消化成,这世上天天都有人被割(开刀),可是我还是不想被割. 我想每个人的心灵深处都会有恐惧的一处,至少我有.

在别人有伤心事的时候,那些可以触动心灵的话语,其实对自己是没有什麽作用的. 再次证明"针不刺到肉是不会痛的",我好象懂了.

好象无知才是最幸福的,我还可以吗?

Coffee and Honey

Have you ever hate drinking coffee due to the sour taste?
I have just figured out that mixing coffee with honey will eliminate the sour taste and make the bevarage perfect to drink. Well, i am not sure if every type of honey works, i use manuka honey. :)

responsibility and obligation

Responsibility is defined as a duty to deal with or take care of somebody
or something, so that you may be blamed if somthing goes wrong.

Obligation is defined as the state of being forced to do something because
it is your duty, or because of law; or something that you have to do because you have promised or because of law.

Mandatory
is defined as something you must do as required by law

I used to think that we have a choice to choose and decide what is our obligation. However, it turns out that life is much more complicated. Sometimes, obligation is not optional, it is the same as responsibility and mandatory.

用心聆听

有一天,有一个老人,在他家的仓库里寻找着一些东西。由于仓库里十分闷热,加上老人的脾气也不大好,所以找了好久才找到他要的东西。
但是当他从仓库里走出来之后,他却突然发现他心爱的怀表不见了!那可是他的传家之宝,也是他最心爱的宝贝!于是他立刻走回去仓库里面,在寻找的当中也不停的埋怨。最后他有找到吗?正确答案,是没有的。
他非常不开心的坐在仓库的门口不停的埋怨,突然老人的孙女走了过来:“爷爷,你在烦恼什么呀?”老人回答说:“可爱的孙女啊,爷爷的怀表掉在仓库里面了,爷爷找了好久都找不到哦。”孙女回答:“不用紧的爷爷,我可以帮你找!”说完就走进去仓库了。
不到几分钟的时间,孙女就拎着怀表走出来了。爷爷十分惊奇,问道:“宝贝孙女!我刚刚找了整个小时都没有找到,怎么你一下子就找到啦?”孙女笑笑回答说:“爷爷,我刚刚走进去仓库里面,我静下心来,用心聆听怀表所发出来的微小声音,过后就朝着声音的方向去找,就这样找到啦!”
这故事的启示给我很大的感受,因为我们生活在忙碌的都市中,许多时候都为工作或为生计而忙碌。或许目前有一份工作,但工作也不是那么的顺心;或许我们有一份收入,但只是供期就没剩下多少了;或许我有个家庭,但却每天奔波而缺乏时间与家人相聚。当人心情不好的时候,就会每天在抱怨这个抱怨那个。
我们何曾有一段时间,用心聆听自己心里面所发出来的声音?想看看,这份工作是不是自己当初所期待的?目前的生活是否应该交给自己安排?我是每天见老板的时间比和家人相处的时间还多?
到底自己要的是什么?目前的状况能够达成你要的目标吗?停止埋怨,用心聆听自己和别人心里面的那句话,找到自己心中宝贝的那只怀表。

abnormal "normalities"

There was time when i was away from family, living outside for the sake of study. Then, i got busy with study, friends and all other activities. 24 hours a day wasn't all that much when you have a lot of tasks to be accomplished. Then, i call home less and less. I used to think that it was okay to take a break from family, because they will always be there for you.

Turn out, i am wrong. Changes occur every seconds. Then, i know i shouldn't take things for granted. Guess i was trying to get away from the problems.

Just realise that all the subtle changes that have taken places to the people and places that used to be familiar to me have accumulated and become a tremendous change. How can i ever miss that?

I realise that i seem to be more and more invisible from their life. I don't really know how to readapt if i were to go back for good. More importantly, my parent are getting older and siblings are getting taller and more mature. And, i wasn't there for them when they need me. Don't know what i can do now, guess i can only go with the flow. Only do what i can right now.

Oh well, it is all about exam associated syndrome again.

Dental tips-Avulsion

I am writing this to share some of the informations i learn from paying thousands to the university. And, i think these informations will seriously help to increase the chance of saving your tooth and your smile. So, take a few minute to read.

Avulsion is one of the common injury that can happen to anyone, especialy someone who are actively involve in sport. Avulsion can also happen in trauma, accident, trip over and in the case when excessive force is applied to the tooth or chin. So, what is avulsion? Avulsion is the total displacement of tooth out of the socket. And, it is importance that you know how to handle the tooth when it is outside your mouth.

A tooth is held in the socket via some elastic connector that we call Periodontal Ligament (PDL) in Dentistry. PDL attach to tooth and socket surfaces that contain living cells that have repair ability. Avulsion leads to complete breakage of the PDL. That means if you insert the tooth back into the socket soon enough, your tooth may have a chance to live by repairing itself. But, the chance for your tooth to live goes down with increased age and time lapse after accident. For example, your top front teeth will have complete tooth and root formation at about 13 yo, if the accident happened before 13 yo, the chance for you to save your top front tooth is increased. In a mature tooth, the tooth will still have pretty good chance of serving its function with appropriate dental treatment, such as root canal treatment.

Ok, Let's talk about how to handle your tooth. As I mentioned above, the surface of the avulsed tooth has living cell and part of the PDL, it is essential to prevent touching the tooth surface with your contaminated hand. Hence, pick the tooth up at the crown (top part of the tooth that you usually see in your mouth).

Then, give the contaminated root surface a rinse with solution of pH and osmolarity that provide optimal condition for the cells to live. The solutions suggested by my expensive textbook are cold milk, saline solution (even you contact les solution), saliva or some special solution that is used to store organs during transplant (but that is not practical, ambulance could have one). The above is the ideal management of avulsion. But if for some reason, you cannot replant the tooth, we have to talk about how to store the tooth.

Store it in one of the solutions suggested above, or in between your cheek. Remember, the time counts. Go to the dentist as soon as possible.

All the measures only apply to your permanent tooth, baby tooth that is avulsed shoud never be replanted. As a general guideline, your permanent front teeth erupt at 7-8.5 yo. So, front teeth that you have got before 7 should be baby tooth. The baby tooth may affect the permanent tooth underneath it-PERMANENTLY.

So, let's talk about some stastic now. It is said that tooth replanted in ideal position within 15 minutes in a person with immature tooth (below 13 yo) has a very high chance to live. Otherwise, there can be healing that lead to inflammation, union with the bone and eventual tooth loss. But everything happen after tooth replantation can be taken care of in the dental clinic. Every problem has a solution. If you are interested in knowing more, you can consult your dentist or even ask me. I will be more than happy to tell you.

Last but not least, i will reinforce the same old statement- Prevention is always better than treatment.

If you are driving, wear helmet or put on your seatbelt.
If you are involve in a sport or any activity that can put you at risk of orofacial injury, take some protective precautions. Ask your dentist for a custom made mouthguard and make sure you do utilise them. Remember children are more vulnerable than adult, take care of them too.

There are more knowledge about immediate treatment in trauma. I will talk about it more in the future.
